Article 1 - Definitions
This information is provided in addition to the information you have already received prior to your treatment day. [This includes the Client Personal Record / FAQs / the Medical Questionnaire and the information provided at your consultation appointments and on the website WWW.hishaisweden.se]. This information has been designed so that you can make a decision whether or not to give informed consent to the SMP™ treatment. Please therefore ensure that you have read and understood this form, the documents mentioned above and the information provided to you during your consultation appointment(s). The purpose of this form is to document the information provided to you, before you decide whether or not to proceed with SMP™ treatment. It also documents your decision whether to proceed with the treatment in the light of the information provided. Only sign the form if you are completely happy to proceed with SMP™ treatment. You have the right to change your mind at any time, including after you have signed this form.
The SMP™ treatment is an elective treatment and there is no medical reason that requires a client to proceed with the treatment.
The SMP™ treatment is a form of tattoo and uses a pigmentation procedure to apply colorants just below the skin in the dermis, in a similar manner to traditional tattoos. Like traditional tattoos, the SMP™ treatment is permanent in nature and, although the treatment can be reversed, the markings can be difficult to remove and the success of any reverse procedure cannot be guaranteed
The colorants used in the SMP™ treatment are made with natural ingredients (such as beta-carotene and iron oxide). Although unlikely it is possible for clients to have an allergic reaction to the colorants used and we can carry out a patch test to determine whether a client is likely to develop an allergic reaction to the treatment if required or requested.
Clients may experience some discomfort (or even pain in rare cases) during the SMP™ treatment. Clients should be aware that there are potential risks and short-term and long-term side effects of SMP™ treatment such as (but not limited to) scarring and discolouration.
As with any tattoo treatment there is a risk of infection, although serious infection is very rare.
Clients with a history of skin cancer, sun damaged skin or any other relevant medical condition should consult their GP / Physician before undergoing the SMP™ treatment. In these circumstances, we may require a signed certificate from the client’s GP before proceeding with the treatment.
The final outcome of SMP™ treatment is very dependent on the individual healing process of each client. Pictures and images of potential outcomes shown prior to treatment are a guide to the results achievable and do not form a guarantee of individual outcomes or results. HIS will as far as possible provide consistent coverage however certain areas of the scalp can appear darker /lighter than others due to skin condition, scarring etc. We will as far as possible provide consistent follicle replication however as with natural hair certain follicles can appear larger than others.
To obtain the best result, the client must comply with our aftercare advice as described in our literature and must attend any follow-up appointments advised by our staff.
If clients tamper with the SMP procedure whilst undergoing treatment sessions this will void the HIS guarantee against fading and/or the original contract with HIS.
Article 2 - Cancellations, payments and complaints
Appointments & Cancellations
We will notify you with the time and date of your treatment appointment. On receipt of this notification please confirm your attendance. Failure to do so may result in reallocation of the appointment time to another client. All practitioners at HIS are trained and approved in accordance with the company's treatment protocols and therefore in the interests of clinical audit, HIS does not guarantee continued treatment with a named practitioner. If you are unable to attend your appointment we require 5 working days’ notice in order to reallocate your appointment to another client. Please arrive for your treatment at least 15 minutes in advance so that the necessary paperwork can be completed. Late arrival may result in reduced treatment time or forfeiting of the appointment. HIS will endeavour to ensure that your appointment runs to time, however, for reasons beyond our control, we may need to cancel or postpone your appointment at short notice. In this unlikely event, we will make every effort to contact you in advance. We do not offer compensation if we cancel your appointment. Prior to your appointment we will inform you of any preparation required in advance of your treatment. Failure to comply may result in cancellation of your appointment, reduced treatment time, or additional fees being charged.
 
Deposits and payments
A non-refundable deposit of 1000 SEK will be charged to secure your booking, should you cancel or fail to arrive for your treatment the non-refundable deposit will be used as a cancellation charge. Should you wish to change your appointment we require a minimum 5 working days' notice. This applies to all bookings. Payments – HIS will accept the following forms of payment:
Cash
Debit Cards
Credit Cards – a 3% surcharge is payable for all credit card transactions
Bank Transfer (An additional charge of £10.00 will be added for International transfers)
Bankers Draft (No person cheques will be accepted)
Payment can be staged over your sessions BUT 50% of your overall treatment charge is payable prior to your 1st treatment commencing. The quotation we have provided you with is valid for 30 days following date of issue. The non-refundable deposit will be deducted from your final treatment session payment.
Treatment suitability
We always assess whether treatment is suitable for you, or likely to be successful, prior to any treatment being carried out. If not, we will inform you as to the reasons why.
Liability
HIS will not be liable in contract, tort or otherwise for any economic loss (including, without limitation, loss of profit), or for any other special, indirect or consequential loss or damage arising out of, or in connection with, it's provision of any goods and/or services to the client. It is the clients responsibility to ensure that he or she provide HIS with all relevant medical details prior to each treatment. HIS will not be liable for any damage that occurs as a result of the client's failure to disclose such details. The client agrees to comply with all instructions and/or recommendations given to them by, or on behalf of HIS regarding the care of a treated area. Nothing in these terms of business shall exclude or limit HIS liability for death or personal injury resulting from HIS negligence.
Your right to complain
HIS endeavours to treat all its clients appropriately, compassionately and fairly. If, however, you have an issue with any matter in relation to your treatment at HIS, you are entitled to lodge a complaint, either verbally, by telephone or in writing. If you require assistance in making your complaint HIS will be pleased to help. The member of staff who initially receives the complaint will convey the details to the Manager and you will receive acknowledgement within seven days of lodging the complaint that an investigation into the matter is underway. During the course of the investigation, HIS may require you to attend an additional consultation with the practitioner involved in your treatment. If you are not satisfied with this initial attempt at resolution, or have any objection to being seen by this practitioner the directors will review your case. If the complaint cannot be resolved locally, the matter will be referred to the directors, where your complaint will be again acknowledged formally and an appropriate solution will be proposed. Article 3 – GUARANTEES
We do not guarantee the result of your SMP treatments. We recommend all clients to do minimum 3 treatments in order to get a better result. Any alterations to your agreed style including repositioning of hairline, increasing density, changing colour, fine tuning and adding side profiles will be charged at an hourly rate + the cost of consumables + local taxes. Should laser removal be necessary the client will be responsible to deal directly with the laser removal company and bear all costs involved.
 
Article 4 - ACKNOWLEDGEMENTS
I confirm that I have read and understood the information set out above and in the documents provided to me. In particular I acknowledge that the SMP™ treatment is a permanent change to my appearance and that there is no guarantee that the markings can be changed or removed at a later date. I understand the efficiency of the SMP treatment varies from individual to individual and I understand that a small percentage of clients may fail to respond fully to the treatment and I as an individual may not respond. I understand SMP treatment will require multiple sessions (minimum of 2) due to the different skin type, hair tone, some clients will require more than one course of treatment and maintenance will also be required I understand the SMP treatment I receive will be appropriate for my specific needs and will be performed by an appropriately trained member of the HIS team. I understand a patch test may be necessary before commencing SMP treatment. The patch test is no indication nor is a prior treatment an indication that you will not have any problems during the treatment. I understand following treatment the skin may be red and there can be a small amount of swelling this is normal. Blistering or crusting is not common but can occur and can take weeks to resolve. I understand that if this occurs I will contact HIS in the first instance. I understand there is a risk of micro scarring although this is very rare. (I will adhere to the aftercare advice provided by HIS following my treatment). I will follow any instructions/recommendations given to me by HIS regarding the care of the treated area prior to, during and/or after treatment. After treatment, treated areas should not be picked, scratched or traumatised and should be kept well moisturised as advised in the aftercare recommendations. I understand following treatment there may be hyper pigmentation or hypo pigmentation (marked lightening / darkening of the skin). Whilst these reactions are not very common there is a possibility they can occur. I understand that following my course of treatment, it is highly recommended that I should wear sun block. HIS will not be liable for any damage caused by my failure to follow HIS instructions/recommendations regarding care of the treated area prior to, during and or after treatment. I confirm that I have advised the HIS Representative of (i) any skin condition that I have in the area where the SMP™ treatment is to be carried out, such as (but not limited to) acne, scarring (Keloid) eczema, psoriasis, freckles, moles or sunburn; and (ii) any medical condition that I have or medication that I am on, which may affect my suitability as a candidate for the SMP™ treatment. I understand I must ensure that HIS staff are informed of any changes to my medical details prior to each treatment session taking place, including any medication or herbal remedies I am taking. I will inform HIS if I am actively sun tanning. HIS will not be liable for any damage which results from my failure to ensure that they are informed of all relevant medical details prior to treatment. I confirm that I have been given the opportunity to discuss the SMP™ treatment with the HIS Representative and I have had the opportunity to ask questions relating to this treatment. I confirm that I am over the age of eighteen (18) and have provided proof of my age, if necessary. I confirm that the information I have provided to HIS Global Holdings Ltd is true and accurate to the best of my knowledge and that there are no other matters which may affect my suitability as a candidate for the SMP™ treatment. I understand that photographs of my face and scalp will be taken before, during and after treatment and stored by HIS for the purposes of providing me with the SMP™ treatment and for its internal records. I understand that these photos will not be used for any other purposes without my separate written consent. I understand I can request a copy of the photographs at the end of my treatment. I consent to the storage and processing of my personal data (including any photographs) by HIS Global Holdings Ltd for the purposes of providing me with SMP™ treatment. I understand when attending appointments I must have prepared the area to be treated in accordance to the advice received at consultation and if I have a suntan I may need to have another patch test performed at my cost and this process may delay my treatment.
